New Facilities Use and Student Waivers Forms

The district has received and implemented new guidelines concerning public use of Board Owned Facilities and equipment as well as waivers for student participation in athletics in light of the current pandemic.  The form for facilities use can be found here (click link) and the student waiver form can be found here (click link) the forms will be made available in Athletics Final Forms Instance.  Students and or families will be required to agree and sign the waiver in order to participate. This guidance was developed by the OSHAA and adopted by the district for the safety of our students, staff and the public in general. Student athletes and students involved in other summer activities on campus must download, print and sign the forms along with their parent signature and return them to their coaches / advisors ASAP or their participation will be denied.
