According to Ohio State law, all students entering school must have the following immunizations completed.

• DTaP-  5th dose required if 4th dose is given before 4th birthday.
• Polio-  4th dose required if 3rd dose is given before 4th birthday.  All Kindergarten students must have 4 doses regardless.
• MMR- 2 doses required.
• Hepatitis B- 3 dose series required.
• Varicella- 2 doses required for Kindergarten and grades 1-4 have to have had 1 dose.

According to Ohio State law, any student who does not have these immunizations completed may be excluded from school until doses are completed. All students entering 7th grade must have another dose of Tdap or Td before the first day of school.

According to the Ohio State law and Alexander Local School District policy, all medications must have a doctor’s prescription and a written authorization from the parent, before any medication can be given at school.  This policy includes both prescription and over-the-counter medications such as Tylenol, cough drops, skin crèmes, etc.

If you feel your child needs these medicines and you do not have a doctor’s order, you can come to the school and give them to your child.  All medication given to your child by school staff must be sent to the school health clinic in original container.  There should be either a note from the Parent or Guardian telling the school health aide how many pills are in the container or the container must contain the amount stated on the container.  If your child needs an inhaler and needs to have it with them at all times, a doctor’s order stating this is needed.   Students found carrying medications without a doctor’s order will be considered to be in violation of the policy and appropriate disciplinary action will occur.  Please understand this is a serious situation, and every effort needs to be made to insure the safety of your child as well as the safety of other children. If you have any questions or concerns please feel free to call the clinic at 740-698-8831.

A copy of the 'Authorization for Administration of Medication' form is available below and may be printed off.  If your child needs medication at school take this form to their physician and have it filled out and sent into the health clinic at school.  Make sure that both the physician and parent or guardian signs the form before it is sent to school with the medication.

The Lice policy for Alexander Local Schools is nit free.  This means a student can't return to school until all live lice and nits are removed from hair.  If a student is found to have lice or nits they may not ride the bus until they have been brought back and cleared through the health clinic.  There are 2 mass screenings preformed throughout the year.  Classrooms will be screened randomly at different times throughout the year.  If a student is found to have lice or nits the entire classroom will be checked and notes sent home making parents aware of this.

10 steps for getting rid of head lice

1. Check all family members and use medicated shampoo on any person that have evidence of lice(live lice or nits).  Since the shampoos are pesticides, it is recommended that only those members of the family who are infested be treated.  Use a magnifying glass if necessary.  The eggs are silver to brown and about the size of a needle.

2. Examine each family member's hair after shampooing and removal of nits.   The nit combs will not remove all nits, therefore picking nits out must also be done.

3. If you suspect resistance do not continue using the same product.

4. Follow manufacturer's directions carefully.  Remember the chemicals are pesticides and have the potential to be toxic if not used carefully.

5.  Treat all worn clothing and bedding used by washing in hot soapy water or dry cleaning.  Include coats, hats, scarves and mittens.  Dry on high heat.  Eggs and adult lice are killed in 10 minutes at 125 degrees. 

6. Seal all articles you are unable to wash such as pillows and stuffed animals in a plastic bag for two weeks or place in a dryer on high heat for 20 minutes.

7. Soak combs and brushes in hot soapy water 5-10 minutes (150 degrees) or a medicated lice-killing shampoo for one hour.

8. Vacuum carpets, upholstery(including car) and mattresses thoroughly.  A louse can live up to two days away from a human host.  Discard the vacuum bag promptly.

9. Continue to check all family members every three to four days for several weeks.

10. Notify all persons who may have had physical contact or shared lice carrying objects (combs, brushes, hats, etc.) so that they can check themselves and ensure that the chain of transmission is broken and re-infestation doesn't occur.
